A provider’s honest breakdown.
“Should I do laser or microneedling?”
This is one of the most common questions I hear- and it’s also one of the most misunderstood. The reason is simple: these treatments are often presented as interchangeable, when in reality they do very different things.
Choosing the wrong one doesn’t just delay results- it can prevent you from getting them altogether. While I use both frequently in my practice, I ensure that I recommend the appropriate treatment for what the patient is trying to achieve.
What Microneedling Actually Does
Microneedling is a collagen induction therapy that works by creating controlled micro-injuries in the skin. These microchannels stimulate your body’s natural healing response, increasing collagen and elastin production over time.
In my practice, I often enhance microneedling by combining it with exosomes, secretome or other regenerative serums to further support tissue repair and improve overall skin quality.
Microneedling is particularly effective for:
- Early signs of aging
- Fine lines
- Mild acne scarring
- Enlarged pores
- General skin maintenance
However, one of the most important things to understand is that microneedling produces gradual, subtle results. It requires a series of treatments and ongoing maintenance to see meaningful improvement.
What Laser Treatments Actually Do
Laser treatments, on the other hand, are designed to target specific skin concerns at a deeper and more precise level.
Depending on the type of laser used, we can address:
- Sun damage and pigmentation
- Redness and vascular issues
- Skin texture and tone
- Fine lines and wrinkles
In my practice, I use a range of energy-based devices and laser technologies, selecting the modality based on the patient’s skin type, concerns, and tolerance for downtime.
For example, non-ablative lasers such as Fraxel are ideal for improving tone and pigment with minimal downtime. CoolPeel on the other hand is better for targeting uneven texture, pores and superficial sun damage. Then there are more aggressive laser treatments such as Helix and CO2 treatments which can induce tightening, target lines and wrinkles, and provide even more of a result but with potentially longer downtime.
Why Most Patients Need Both
One of the biggest misconceptions is that you have to choose between laser and microneedling. In reality, they serve different roles in a comprehensive treatment plan.
Microneedling is best used as a maintenance and collagen-building treatment, while laser therapy is used for correction of visible damage.
For example, a patient in their early 30s with relatively clear skin may benefit from regular microneedling to maintain collagen and prevent early aging. However, if that same patient develops sun damage or uneven pigmentation, microneedling alone will not be sufficient at improving the overall appearance of the skin, which is where laser treatments become more necessary.
The Biggest Mistake Patients Make
The most common mistake I see is patients choosing microneedling because it feels safer, easier, or more familiar- even when their concerns would be better treated with laser.
This often leads to frustration when results are minimal, not because the treatment doesn’t work, but because it wasn’t the right tool for the problem.
How I Decide What to Recommend
When I evaluate a patient, I’m not thinking in terms of trends or popularity. I’m looking at:
- Skin quality
- Presence of pigmentation or vascular issues
- Degree of collagen loss
- Patient goals and timeline
From there, I create a plan that may include microneedling, laser, or- most often- a combination of both.
Choosing the Right Treatment for Your Skin
There is no universally “better” treatment- only what is appropriate for your specific concerns.
The key is understanding what each procedure is designed to do and applying it strategically within a larger plan.
